Preparing for a Visit

A number of things can be done by the patient to prepare for the first visit that can make the experience much easier.  It is important that the patient become an active member of the health care team.  Here are some important recommendations. 

Please bring the following with you to your appointment:

Health insurance card
Co-Payment (due at appointment)
Referral forms (if applicable)
A list of the physicians and addresses that you want reports of the consultation sent
Recent medical records including operative reports, pathology (biopsy) reports, and radiology reports, lab tests
Recent radiology films including x-rays, mammograms, CAT scans, MRI scans and ultrasounds (dropping these off prior to your appointment for review will save time)
Pathology slides for review
Please check with our secretaries that all necessary information has been received prior to your visit
A list of all your medications and allergies to medications
Write down questions prior to your consultation
Feel free to bring a family member or close friend to the appointment to take notes and help ask questions
